Why does my fish keep dying?

My lawnmower blenny keeps dying. Everything is fine. Temperature, pH, air circulation, etc. I have tons of algae in my tank, so thats not a problem. Maybe I'm not acclimating properly. I only float the bag. Currently, I have 1 starfish and 1 clownfish and they're doing fine. Any suggestions would help.
