How to interpretate an incident report?

If let say the frquency rate is 2.01 and the severity rate is 42.26 how to intepretate the value whether the figure show that the performance is good or not. some of the formula i see is per million people and some just 100000. which formula to use. is there any standarr bar to compare with whether the figure acquired ic good or bad. thank you.
